Dod ID Number for DD3150?

I'm filling in the DD 3150 for upcoming visit and I am not sure what to put in the "Dod ID Number". Should I put my candidate ID, leave it blank, or something else?

I have an upcoming visit next week, and I've looked through my emails and my "Tour Letter" Official correspondence, and I didn't see anything about this form. Is this form a newer requirement for visits? If so where did you/could I get access to it?

Yeah it’s a requirement for visits. I just looked it up on google and the first result was the form. Upload a copy for you plus 1 for each of the people going with you. Upload on the portal but I would also bring a copy with you to your visit just in case

My DS is also scheduled to visit this week. He had received an email from someone in the admissions office reminding him that he and any other visitors (parents) need to complete this form. We did and uploaded it in his portal (the DD 3150 is one of the forms in the drop down menu to upload). Hope this helps.
