I completed my medical exams in March, and there were some issues I had to resolve with the CIV team about my name spelling that delayed it a little. However, now my status is "complete and on hold" and says I do not need to take any further actions. I am worried though as May 15th deadline to keep my conditional appointment is approaching soon. I emailed my coach, Ms. Castineira (medical POC), my AO, and the CIV team (again). I do not anticipate any issues with my file. If I were to need a waiver I am worried there would not be enough time to receive one. Should I take any additional steps?

You said you emailed. Did they respond? In particular what did your AO say? If your DoDMERB is submitted they should list any potential health conditions thath would require a waiver.

Yes, Ms. Castineira said that I should contact the CIV team, which I already have and they said I the doctor's office did not fax over my reading aloud test and did not date one of the pages. The CIV team member I spoke to said that she would call after, but they have been trying to contact this doctor's office for weeks. My AO has not answered my email yet, but my coach forwarded my concerns to the Athletic Director. I am going to go back to the doctor's office Friday morning and see if I can handle the issue and get it forwarded over. I do not anticipate any medical issues whatsoever.

Sometimes walking right thru the door, nicely dressed, smiling, speaking politely, and mentioning Coast Guard Academy, will get a person the results they seek.

I suggest that if they say, "We'll be sure to take care of that", that you politely state that since it is time sensitive, you would like to wait there until it has been faxed so that you can verify it has been done. Once you see them fax it, you could even call the CIV team while still in the doctor's office to confirm that the forms have been received and that they are complete.
