DODMERB Disqualifying Conditions?

Is one instance of blood in urine with a doctors visit involved enough to get disqualified for DODMERB if it was ten years ago and hasn't happened again since? Is it significant enough that it should be included on the DODMERB medical history? Thanks.
 
Is there a question on the form that asks if there was ever blood in your urine?

IMHO blood in urine isn't enough to report as "any other medical conditions". What was the cause or
diagnosis?
 
I honestly don't remember what the cause was. But on question 46 of the questionnaire it says "history of blood, protein, or sugar in urine".
 
This is only my opinion, but if there is a history you would say yes and then explain it in the remarks. State only the facts for example: incidental finding (if it was) of blood in urine once in 2005. F/u urinalysis negative, (if it was). No other problems. Then move on to next item. Be brief, but truthful and supply what is asked for and no more. If it was not incidental, then you may have to explain a bit more.
