Dodmerb herniated disk

I had a slightly herniated disk at L5 S1 about two years back; had physical therapy and a cortisone injection. I'm as good as new now and have no pain. I exercise regularly, lift weights, swim competitively, work jobs, etc.
I haven't yet received a scholarship offer (AROTC). I was wondering if I should go to my orthopedist and request a letter stating that I'm good to go before I show up for my exam. Would this help prevent me from getting disqualified, or will they disqualify me anyways because of the previous herniated disk and make me go through the waver process to attempt to become scholarship viable?
