I am in the process of completing the medical history part of the DODMERB process. Under "Orthopedic Questionnaire" it asks for the date my injury occurred, but every time I type in the date it is automatically deleted. Is anyone else having this issue?
Also on a side note, I have had several surgeries that have involved placing hardware in me. I have a screw in my finger and a rod plus 4 additional screws in my leg? Is there any chance of receiving a waiver? Both my finger and leg are fully functional and there is no chance of additional re-injury.
