DODMERB Questions Confusion

Hello. I'm filling out my DODMERB questionnaire and am confused about a particular question. I answered yes to taking medication to prevent headaches and taking vitamins. The follow-up is asking, how many times has taking medication to prevent a headache occurred? (same thing for vitamins) I can only submit a number. What exactly is this asking?

Our son is filling out the DoDMERB medical history questionnaire and accidentally clicked “yes” on a question when the answer is “no”, now it is locked on “yes” for that question…can it be fixed?

My son was recently awarded the NROTC National Award and just received his Welcome Letter/ DoDMERB NSI Package Checklist. The forms are poor copies. Should we pull the forms from the DoDMERB website? Thank you

@rj$0921 When we updated the attachments on 3/25, we put a gray watermark on DD Forms 2807 and 2808 as an example for students so they understand what they should look like. We also updated our website with a copy of the Mandatory Information Package and FAQs. I have attached a copy of the FAQs from our website and the package, that was emailed out to all Midshipman Candidates, for your reference. I believe questions 3, 4 and 6 will provide the guidance you are looking for.
