Hi! If I take the DODMERB exam one time, will it be seen on my application for everything? Ex. Each SA/ROTC

You only have to complete the DoDMERB process once. It will be initiated by the first SA/ROTC request and then it will be forward as more requests are sent to DoDMERB. Qualified is good for all. The waiver process is independent ie you could get a waiver from USNA but not NROTC.
