DoDMERB Waiver Rebuttal Initiation Form

To initiate the rebuttal process for a denied medical waiver there is a form to fill out. There are three sections: name, disqualifying conditions, and "instruction for medical rebuttal". What is supposed to be typed in the "instruction for medical rebuttal" box? Is it supposed to be instructions for what DoDMERB should look at in your medical reports, or is it something else?

I would think that its the actual reason for rebuttal, why the DQ is incorrect.

We looked on the Dodmerb site and also called our rep - but they didn't know anything about this form. Where exactly did you find it?

It’s toward the bottom of the waiver denial letter from Air Force. Had to type in the url.

I don’t think letter was actually posted to portal. DD received an email “USAFA Waiver Determination Notification”. There was a link to open it (with password)
