Can somebody tell me exactly how in depth we have to be when answering the questions about being treated at a medical facility such as Urgent Care? I do not have all of the times my son has visited or been treated at an Urgent Care in his last 17 years. Are they truly expecting us to list out all those incidents? Thank you!!
 
We used insurance claims to get dates for past doctor's appointments and reviewed calendars to get a complete list.

Yes, your child (not parents) is expected to list out all those incidents.

Parents may assist the applicant.
 
I considered the Urgent Care visits like ER visits. If we went to Urgent Care for strep throat/ear infection, I did not record - I did, however, note that he had a tonsillectomy for recurrent strep throat. And I noted if the visit was for broken bone, etc.
