I'm filling out my medical history and I have a question. When the form asks, "How many times has X condition occurred?", what does this mean exactly? It seems straightforward for something like a surgery or a concussion, but what about something underlying with symptoms (i.e. shortness of breath) or wearing a device like orthotics (which I wear every day)? Should I put down the number of times I visited a physician for said condition/symptom for treatment? Or estimate how many times I experienced that symptom?

You are correct. Examples:

1. I had three surgeries on my left knee ACL...12 May 2009; 27 Mar 2012; 1 Apr 2018. Add the additional details.
2. I had 4 x concussions. 12 May 2009; 27 Mar 2012; 1 Apr 2018, 1 Jun 2020. Add the additional details
3. I saw Dr Schmucatelli, address, my Podiatrist on 15 Apr 2015. He prescribed orthotics for my symptomatic flat feet. The orthotics worked. I no longer have pain and haven't seen him/anyone else for this condition :wiggle:
