Does Anyone know where certain Enrollement Forms should be sent to?

Hey everyone,

Some enrollment forms for USCGA do not have a place to send them to written on the bottom of the forms. This includes the firearm form, the state of legal residence form, and the authenticity of ethnicity form. Also, the CG- 3307 says, "Email copy to CG PSC -BOPS-C-MR." If someone knows where these forms should be sent to, or the right point of contact it would be greatly appreciated.
 
I believe they are all supposed to be returned in one of the envelopes provided. I believe that email instruction you're referencing is an internal instruction - not for the appointee.
 
Chris McMunn stated to us, all of the for point of contact information and where the forms should be sent are in the Cadet Handbook on the individial forms.
 
Parent using son’s account here: The white envelopes sent by USCGA for use by incoming cadets to mail in their forms are all pre-addressed to the same address. Some of the forms are multiple-paged, and if you have other supporting documents to submit along with the forms (immunization history print-outs, COVID-19 vaccination card copy, foreign national parent’s green card/passport copy print-outs, etc.), the combined pages become too thick (in my opinion) to use the white envelopes. The white envelopes are nice, but they’re a bit small and not very robust. Basically, we did not trust sending these critical documents in the white envelopes. We grouped all the documents together separating individual forms, and when applicable, their supporting documents with paper clips. We placed the combined group of forms inside a plastic paper protector and took them to the post office. We used USPS priority mailer for $7.95. For the mailing address we used the same address as annotated on the USCGA white envelopes. In addition, on the front of the USPS mailer we annotated the following: “CLASS OF 2025 ADMISSION FORMS ICO *cadets’s name*.” Doing that provides Admissions a heads-up of what’s contained inside the USPS mailer.
 
Also don't forget to make a copy of all forms and bring them with you on the first day of SWAB summer. You might not need them, but I believe the instructions tell you to do this and you might find you need them. Also, if it is like last year, the one form for direct deposit says you can make a copy of a voided check. But it turned out that they needed an actual voided check which we had to send to our SWAB to submit. So bring that with the copy of your forms.
