dq'd remedials and waivers

My DS will be applying to the USNA. His history of spondylolysis is a disqualifier. (DQ code D227.80) We have already obtained all his medical records in advance anticipating his DQ. My question is should he see his doctor and get a letter clearing him of any ongoing physical conditions and have that with him for the DODMERB or will there be a specifiic form that that doctor will need to fill out after his exam.
 
The doctor will have specific paperwork for the DoDMERB exam but you can be proactive and start collecting documentation for everything to do with your son's condition- medical records, diagnostic tests, etc. You will probably need to supply all of this for a waiver. Time is of the essence so it is a good idea to gather all of this ahead of time, and to schedule the DoDMERB exam at the earliest possible time.
