Exam Packet and Audiologist Exam

I understand that I need to give my exam packet to the medical exam. I have also heard that you don't bring it for the eye exam, but I'm not sure. What do I do about the Audiologist Exam? Is there a certain part of the packet that I need to bring to them?
 
Medical History form = Applicant must bring with them and their picture ID.

Nothing but your picture ID card must be brought to the Optometric exam.

Very few folks have to go to a separate audiology. Those that do, must bring their picture ID.:wiggle:

So who fills out the parts such as "Audiometer," "Valsalva," and "Read Aloud Test?"
 
Audiometer = Audiologist
Valsalva (probably will NOT be performed)and Reading Aloud Test = Physician:wiggle:
