Eye Exam Forms at Appointment

Here's a timeline of events:
  • Last week I was assigned an eye care center to complete the DoDmerb Eye Exam
  • I scheduled an appointment with them for next Friday 9/25 and reported this on the DodMets Website
  • A couple days after scheduling I called the eye care office to confirm that they had received the necessary forms to complete a ROTC/Service Academy Eye Exam and they said they had not (on the DodMets checklist it says that the eye care provider will have these forms on the day of the exam but I wanted to check this)
  • I called the DoDmerb phone number on their website and left a message with my question, leaving my email and phone #. Shortly after I received an email response that the forms would be sent in the next thirty minutes
  • I waited till the next day (today) to call again and they said they have still not received the forms

My question is whether there is something I'm missing on how to send these forms out and if so what that is? I saw that if two exams go by without you completing the exam for whatever reason (forgot paperwork etc.) then you will be disqualified and I obviously do not want this to happen.
 
my daughter went to her eye exam yesterday and did not confirm if they had the forms but she is assuming they did bc provider told her she was great and all looked good and he would send the documents right over.
