Does anyone have the link handy for the form 86? I'm at work and don't have it with me, wanted to print it out so we could get started on it.
 
What is form 86? Did it come in the mail? I just want to be sure everything is sent in
 
You should have received an envelope from the Department of Naval Science. It contains a fingerprinting form to be done at your local police station along with additional instructions on how to download form S86. This is a 20 pages national security questionaire that must be completed and brought on Indoc day.
 
I think my son mailed may have mailed the form. He will have to bring the copy on I-day finger prints are on tap for next week.
 
Did he download the form? There is a short form (it has the Academy logo) that comes with the medical forms, don't confuse it with the lang form (S86) which must be dowloaded an filled but cannot be saved. You better double check.
 
Make sure to save a copy of the completed form for the rest of your life in a safe place. That thing will come in handy many times in the future, especially if you ever have to fill one out again.
 
Thanks we have copies of everything. Now we need to remember where they will be filed.
