Hey all, my DD is in need of DD2351 and DD2492 to complete her application to The Citadel, backup plan as she waits to hear from the academies, does anyone know where she can find them? She says they aren't on her DoDMERB page... Thanks, Todd

Quick google search brought them both up easily
 
The DD 2492 is the DoDMERB questionnaire that your DS completed to apply to the service academy DoDMERB.
The DD 2351 is the DoDMERB Physician's report that the DoDMERB doc completed after examining your DS.

Don't re-invent the wheel. The Citadel simply wants copies of what you filled out and what the doc completed.

Yes Sir I understand that, we were not given copies with signatures after the exams. Therefore she's trying to get them. We figured they be in her DoDERB qualified file.
 
Have her contact DoDMERB or the physician to get copies. Hopefully you kept a copy of the candidate questionnaire.

Fiver years ago, DS was accepted to El Cid but enrolled elsewhere. If I recall, The Citadel only needs these forms before she actually reports to Charleston, so you have time.
 
WRONG ANSWER: The new forms are 2808 and 2807-2 that are obtained on the DoDMETS website AFTER she is downloaded to us as a valid applicant
She should comply with the sticky FAST on the DoDMERB tab on SA Forums. I will reply Sunday

Apologies. Misunderstood what you were asking.
 
No worries. I work this everyday. Wouldn’t expect you to know this.

I think she’ll find she doesn’t need any forms at all. After she contacts me, I think she’ll find that she will ask us for a copy of her SA exam to be used for the Citadel...but I won’t know that for sure, until she contacts me and I see her specific circumstances
