Hi all,

I’m completing the medical history exam on the DODMETS website. I answered “yes” to having a history of wearing glasses, which prompted the following question:

“How many times has "Worn/wear contact lenses or glasses (Bring your eyeglasses no matter how old they are.)" occurred?”

For this question, should I enter the number of times I’ve had to get a different prescription for my sight throughout my life?

Also, it asks me for the diagnosis. Should I just write what my prescription was?
 
That follow-up question in the DODMETS website is odd to ask an applicant.

I would just say something like, “I have worn glasses every day since 5th grade.” Or “I’ve worn glasses every day since 6th grade until 9th grade when I started wearing contact lenses.” Something along those lines should answer the mail.

For diagnosis, I’d say something like “I wear glasses for nearsightedness (or farsightedness).” Leave it at that.
 
I put in the date of my first appointment with my eye doctor, then for diagnosis I put myopia (nearsightedness). I did not put down my exact prescription, I don't think thats a big deal because you have to go to an eye doctor anyway.
