I was just wondering if West Point will ask/care if you’ve ever seen a therapist? Though, I never actually had an appointment with one, I registered to one when I was in eighth grade, but some technical malfunctions in their system, gave me time to rethink that decision. Would it be on my record?

No, I am pretty certain that there would be no record unless you actually saw a therapist and there were documents about the session.
For clarity, West Point would not ask this question. The question would be on the DoDMERB questionnaire which is administered by the DoD out of Colorado Springs, at the Air Force Academy (They service all the SA's and ROTC programs).
