Immunization Form

I am also in this boat. Got all the required immunizations done except the Hep A/B combo shot and the polio booster. Planning on getting those done next week because the regular care physician didn't carry them. Plus, there was some issue with getting my blood results back, that caused a delay.

However, I am going to ask if I could just bring them in with me when I go up on the 22nd of this month. Hopefully that will be fine.
 
As a pediatrician, I find the difficulty that some of you are encountering with your own physicians disturbing. Prevention and immunizations is what we do. My son's form was completed and his shots updated quite easily by his doctor's nurse.:smile:

Varicella Titer

My problem is that the Dr. had to send the blood work for the Varicella Titer to a lab in another state. It has been two weeks and the results have not been returned.
 
We also send routine labs out of state to LabCorp in Birmingham, AL. I still got my son's varicella titer and blood type back within 48 hours. Two weeks seems like a long turn around time.
 
The immunization form says that the adult dose of the Polio Virus Vaccine (OPV,IPV) has to be administered after the 18th birthday. Since DS will not be 18 before this form is due (April 15th), I'm guessing I should send in an incomplete form and then re-send another form with the update it after he receives the adult booster in another month or so?

I called the head nurse at the WP clinic. She said go ahead and get it completed in the "birth month". Call the number on the form and they will let you know...
 
Does anyone have the USMA 40-1 form - the form that you record the immunizations and the blood type on - I had one but the doctor's office put the immunizations in the wrong order and I wanted to print the form again and get signed.
 
My DS accepted his nomination March 9 but hasn’t received any instructions for candidates yet.. do you recommend asking someone about this? Getting a doctors appt in our area can be tough.

Hi, I do not have the 40-1 form but while trying to search for previous year’s immunization requirements for USMA, I found a very out-dated form that said MCV4 (Menactra) was required within 6 months of R DAY. Is that requirement on this years form? Along with the IPV? And possibly the Tdap?

It looks to still be the case for the TB PPD skin test or QuantiFERON blood test to be completed within 6 months, along with MMR and VZV titers?
