Ok, my DS has received all the required shots and TB testing etc. but the issue we have is since this Polio was to be administered this year it was his 5th round, so on the shot record, it will only print out dates for the required first 4. Our nurse has hand written the date on the form and signed at the bottom but I am afraid this might not work. Anyone else have suggestions or similar experiences they could share?
 
Last year DS submitted his shot records with some being hand-written and signed by the nurse. He had no issues.
 
My son's immunization form was filled out by hand and signed at our Doctor's office. He didn't have a problem. Check if there is a contact number with the directions on the medical forms. If not, your son should contact his admissions adviser and ask. If they don't know, they will tell you the proper person to contact.
 
When the topic of immunizations appears, it bears repeating. Have an extra copy of all immunization records with the appointee during I-Day. It happens every year, that the Academy will not have the records and rely on the hand carried copies to keep from getting reinoculated unnecessarily. This happened to our son and it saved him from getting stuck again.
