For the PTR packet, there are immunization record forms that need to be completed by a healthcare provider due May 15th. What if I am unable to schedule an appointment with my provider before then?
 
- Are there other providers in the practice or network who can do it?
- Can a nurse administer it?
- Does your healthcare plan have a partnership with a retail pharmacy such as CVS or Walgreen’s for immunizations? I can’t recall the last time I received an immunization at my PCM’s office. I get them at Walgreen’s.
- Does your county or city have a public health department that provides immunizations?

If you show up without, you’re likely to get port and starboard arm punctures, always a delightful foundation for I-Day evening push-ups.

Think outside the box to problem-solve.

you dont need to get an appt to get the form filled out - just send in your portal or fax or however your dr's office does them. ours even charges a fee to fill out the forms - but you cant attach vaccine records - the form needs to be filled out by the nurse and signed by the dr IF the form is the same as my now youngsters. This advice is IF you have received all the vaccines needed - if you have not then you need to get them somewhere like a pharmacy, urgent care or public health dpmt like Captain MJ says above!!!
