Incomplete-closed status

Hello,
I have been struggling to finish the health check for a while and was wondering what to do at this point. At first, they said that my heart rate was too high, so I got it remeasured at my school with our nurse. I sent them the new heart rate results using our school letter head as requested and a signed document where my parent's signature had been missing. My heart rate was under 99 this time following the standards. I asked if this was all they needed, and the person on the other side said yes. After about a week or so, I got another email saying that they were missing another document that required my parent's signature. I emailed them and was told that I needed to submit the same exact material I had previously sent. The Concorde person has been responding but rarely answers my questions. What should I do?
