Is every candidate authorized a medical exam, or only the competitive?

I am asking strictly due to curiosity, as I find it hard to believe that the government would pay for every person's medical exam, even the not competitive.
 
You have to have your file complete before a medical exam can be conducted, so I assume that if you were neither competitive or admissible status, you would not receive a medical exam.
 
I assume there is some minimum requirement, but the key is to complete a large part of your package. When my son completed his essays earlier this week, it triggered the medical section the very next day. The essays were the last thing he needed to complete in the top section. Think of it this way too. If you were running a business when would you schedule people for medical exams? It cost money to give the exam. All the serious applicants need to be tested.
