Once I accepted my appointment, one of the required items was to fill out the SLGI life insurance form. The instruction kit says to bring the filled out form to R-Day but do I do anything before that to make it turn into a green checkmark?

No, it stays red until R-Day. Some people managed to get it turned in before R-Day, but it's a waste of time, and you'll most likely have to redo the form during Beast, anyway.
