LOE - overnight visit

According to the candidate portal, overnight visits are reserved for accepted candidates, LOA or LOE holders. Do the LOE's specifically state an invitation to an overnight visit or do you have to read between the lines? Is a letter stating that one is 2Q and just waiting for a medical waiver considered an LOE? Has anyone been successful in obtaining an overnight visit without these factors?
 
The LOE letter itself in the portal includes an invitation to an overnight visit and a link. And I think it is under official correspondence with the title “letter of encouragement.” If that is not what you have, you could always reach out to admissions and request a visit and see what they say.

No. Neither a letter indicating that one is 2Q (academically and physically qualified) nor a letter indicating that one is 3Q (academically, physically, and medically qualified) are the same as a LOE (Letter of Encouragement). A LOE has no specific benefits other than granting permission to visit. LOEs tend to be given out early in the admission cycle to encourage candidates to complete an application. In all likelihood, your RC will have you wait until you receive an appointment before granting an overnight, especially now that we are into the last couple of months of the main appointment season. For better or worse, USMA is a bit more discerning when it comes to overnights than USNA, which tends to grant CVWs pretty readily when requested (by an admissions officer or BGO).
