Medical History Survey

I began filling out the medical survey, and one of the questions was if I have asthma or a history of it. I had asthma until I was 5 years, but it was nothing serious, and I have not been diagnosed with asthma since then, nor have I had any sort of breathing problems. Do I say yes or no for the survey. I would assume yes, but there is not a place for an explanation, so I just want to make sure.
 
Mr. Mullen will be here shortly, but I believe you've already answered the question yourself. It asks if you've had a "history of asthma." Explanation and context will come later (including the fact that you had it when you were less an 5 years old.) Search in the DoDMERB forum - there are several threads that you will probably find helpful.
 
Ok thank you-it then asks exact dates for when I "had asthma" and we have no idea because it was 13+ years ago so my mom does not know exact dates for when I had problems so we are unsure how to answer those questions
 
The question is "Currently have or any history of" - Therefore there is a history and the answer should be yes. Also, not only is there a space to explain, you can't proceed without an explanation. Dates? If you don't know exact dates, an estimation and using the first day of the month...EXAMPLE: 1 Aug 2008 :wiggle:

Thank you! I was trying to be very precise/accurate but if an estimate is ok that is a lot easier. And I asked too soon I found the space to explain after the fact. Thank you for your help ....enjoy your evening!
