My daughter's pediatrician retired and I do not have access to her files through age 10. Any suggestions? He was not part of a big group.

Because the last four digits (not the whole number) helps Mr. Mullen (The Deputy Director of DoDMERB) find your daughters file and to verify her.

If you are not convinced, go to the DoDMERB website, and click on the button for "Questions on the Process.' on the left margin. When you get to the questions list, on page 11 you will see Mr. Mullen's email which is the same one as above.
