Missing Permit to Report Forms

Does anyone know where I can find the NHCA Immunization General Consent (BMU and BHC) form? Also is the Mantoux Tuberculin PPD form included on the Immunization Record or is this a separate form?

I think they are both part of the immunization form. I had to schedule a doc appt to get the form filled out because it says to be filled out by primary health Provider. You have to get the ppd test done unless you have had one after Jan 1 of this year.

I found a copy of the NHCA form online and put it in a cloud:

The PPD form is a part of the Immunization Record Form but if you opt for the QuantiFERON test you attach the documentation to that form and send it along with it.
