Need to send in letter and medical history

My son was told he needs to fax in his medical history from his family dr. But we were not given a fax number to fax it to. Does anyone know where we can find it? Son has been calling all week and not getting anyone to answer.

I'm honestly not sure all I know is my son called and said I need to fax his medical records so they can verify he does not have psoriasis.

As he was dq'd for it because of a question on his medical history we answered.

You may be talking about a remedial (asking for further information on something you answered Yes to on the medical history exam).

For my two remedials, I scanned all of the paperwork into two PDF files on my computer and then emailed it to my assigned technician on the DoDMERB website.

Best of luck, make sure to include everything in the paperwork you send in!
 
On your son's DoDMERB website there should be a letter requesting the additional medical information. My dd had 3 options: send by mail, PDF, or fax. The contact information is in the letter.

Ok I looked at screenshot of his DoDMERB it shows awaiting receipt of remedial but dont see a letter where I can get the info but the dq is for psoriasis which he does not have so would be sending his medical history ok or should he get an appointment with a dermatologist to confirm no psoriasis
 
@HawkeyeMom and @WD1 For those of you that had to send in an AMI, how long did it take to get a response? I sent my AMI in early September and it still says "awaiting receipt of remedial."

It only took a day or two for my son. You may want to contact your DoDmerb liaison. Hopefully they have it but never updated your portal.

I have emailed it over 4x, left a message for my liaison with no response, emailed and contacted the help desk with no response and last night I sent an email to the man in charge. I really do not know what is going on.

DD has only sent part of her information in (medical records). We are still awaiting MD visit, so not complete... However, when dd had questions, she emailed her DoDMERB technician and received a response the next business day. I would email your DoDMERB technician, assigned on your remedial letter as the point of contact. Make sure you include the last 4 of your SSN, as the technician informed my dd that she needed that to access her documents/respond to her questions accurately.

I did all of that. Complete radio silence from everyone there. Technician and help desk. Name with last 4 of ss on subject of email as well as body of letter as directed. All i's dotted and t's crossed. Absolutely nothing.

That is very frustrating! The next step, I would think, would be to contact Larry Mullen, who is in charge of DoDMERB. Or your admissions counselor to whatever service academy (s) you applied to?
Have you tried a very simple email, like"I am following up to see if you have received my requested AMI information that I sent on X date?" So your technician can answer with a simple yes/no response? They are so busy now that I think longer emails are more difficult to respond to.

Mystery is solved! My emails were being sent through my school email - ________.__.k12.us account which apparently will be BLOCKED by either my school server or their server. I sent a follow-up through my gmail account and got a response in 10 min. It is now UNDER REVIEW! Wow, how frustrating! Good grief.

Something similar happened to us.
My wife, who is employed by a health care provider, attempted to send DS's info via work email to DoDmerb. After multiple failed attempts we learned that her work email blocks medical records to prevent potential HIPPA violations. You may have been a victim of the same software.
 
I feel like this board needs to put all of this out there. Or Dodmerb. If you do not hear from us within 24 hours then something is wrong type of thing. The reason why I say this is because so many people on this board say "hurry up and wait" and "be patient" which is what I was doing. Well, I could have had this all taken care of 2 months ago had I known!
 
I know this has already been resolved, but if you just read the entire letter instead of stressing and jumping to "oh my god what do we do" mode, it has almost always been ridiculously clear where/who to send information to. They even bold the email and name of the technician and his purpose at the bottom of every letter.
