No Send Direct Deposit Form?

I was about to mail my direct deposit form when I ran into this PDF file on the USAA website:

Take a look at section 3 of the first page. Everything is already filled out except for the date and depositor account title. Does this mean I don't have to send it to them anymore?

That is identical to the one that my DS filled out and sent to USAA...I am assuming that you fill in your details, sign and bring with you to R Day for processing

I just called USAA. And the representative said that they have to pre-fill it out. So she filled everything out besides the signature, gov name/agency, etc. then put it on my online documents. Then you print/fill out and take it with you to R-Day.

tl;dr Call USAA and ask. They are very helpful.
