When you are supplying the name and address of your counselor and math teacher for the evaluation form in the application, do you provide their personal address or is it the school address that you enter in.
 
Where do you get the evaluation form for teachers to fill out in the NROTC application?
 
School address and the recruiter in charge of your application should send it to them.
 
I input my teacher's school email when I filled that section out. The NROTC detachment will send out the teacher evaluation forms to them in the email that they send to them.
 
My DS emailed his teachers and the counselor first to ask them if they would be willing to write the recommendations, and if so, which address, email, and phone number they would like is to list for them (which usually was the school info). As a courtesy, before we supplied their contact information, we let them know whom to expect emails or recommendation requests from.

Really good idea, given that they *probably* are on their summer breaks. Some won’t check work emails. Or wouldn’t know to expect the email and may miss it.

It’s super easy to touch base, and inquire what email they prefer. And also to follow up that it’s been received and returned. Good luck to you!!
 
Does it have to be a teacher from sophomore year or does it have to be Junior year
