If one is medically qualified (i.e. DODMERB letter in hand), do you still need to fill out and submit the preparticipation physical evaluation forms in the NSI packet?

I would plan on doing anything the instructions for the NSI packet say, which is yes.

DODMERB I believe is more for the scholarship side to conform to DoD requirements. I suspect the sports physical is to clear your ability to do strenous exercise at NSI. Just like sports in high school.

Absolutely positively. It is not optional and DODMERB physical is not in lieu of. Make sure you have a sickle cell test lab results, too.
 
Unless they changed the directions/instructions from last year, the DODMERB letter (for ROTC, not a SA) will suffice. The preparticipation physical was for those folks who had not yet cleared DODMERB. Totally agree about the sickle cell test - it was incredibly frustrating to try to figure out if DS was tested as a baby. We ended up just going to the doctor and received a lab order to do it. We had to pay for the test out of pocket, but it was worth it to not have to spend months trying to obtain the medical records from when he was born.

The mandatory check list on page 5 pretty clearly says sports physical. You can do what you want, but I recommend following the instructions unless specifically told by your unit to you that you do not need the physical.
Thank you. We reached out to the CMGO and they confirmed that the pre-participation physical evaluation is required even with the DODMERB qualification. Thanks everyone for your input.
