Optical exam paperwork?

Just looking for confirmation that DS does not need to bring paperwork for the optical exam.

We didn't. The paperwork packet on DoDMETS is for the medical exam.

I did need to sign a parental consent form that the doctors office provided on site.

My DS only needed his ID. The packet of paperwork he downloaded was just taken for the medical exam.

If you wear contacts, be sure you haven't worn them for three days beforehand. I showed up with contacts in and then had to come back later, pretty inconvenient for a 1.5 hour drive.

No paperwork. And my DS's exam lastd 3 minutes,

Thanks, everyone! And thanks for the heads up re contacts @ThomasJ - DS doesn't wear contacts, but that will be helpful to someone reading!

The optical exam is done. ID was the only thing needed. Checking it off the list!

Physical is tomorrow.
