What exactly are the papers you're supposed to bring to each appointment?

Well at least for me, Concorde Inc sent me the information to set up my DoDmets account. After that, I filled in my medical history and then it said "print." Once you make an account, its super easy. They put it into 5 easy steps and you just do one after another. As for the papers, I brought the papers for all of my appointments. But only my medical examiner needed the paperwork, not my eye doctor or audiologist. hope this helps some
