On the parking pass form, Block 25 is for the name of my sponsor, who is my P2B. However, there is also a 'signature field' in that block. There is nothing in the instructions about this field, and when I google Form 5512/1, not a single version has this signature field. I am assuming that my son signs this field next to his printed name, but I don't like to ass-u-me. Would it be for him to sign, or for office use?

If you're talking about this (see below)

Then you just need to put your Plebe-to-be name there. You sign at the bottom. It is for your DBIDS card (to let you drive onto The Yard) so it just requires your signature. If your spouse wants one, it will require a second form. You may bring up to five (5) people onto The Yard in a vehicle, with your pass.

Plebes don't receive their Alpha # until I-Day, so just the name.

They will still need to process you (take your photo and get your fingerprints) at the Gate 1 Office/Visitor's Center, before you are issued your card. Don't plan to drive in, with form in hand. You will not be allowed to park there until you have that physical DBIDS card.

Everything else on the form is the same, including where I sign.

Ah! It changed from last year, then.
Yes, your son signs there. You could tell him that in effect, he is giving you his permission!
How about that?

Is this form a mandatory one? My parents don’t have much intention of driving onto the yard. They’ll Uber whenever they visit and walk in through the visitor center.

It’s handy for bad weather and those lovely Annapolis bricks are iced over or a nor’easter is lashing the Bay. For when they are picking you and friends up to take you to dinner off the Yard. For when they are ferrying you and friends post-football game for a bite to eat and then dropping you back at Bancroft. For when they might be picking you up or dropping you off when you have a lot of stuff. For when the Gate 1 Visitor Center walk gate is closed, and they can use Gate 8.

The form is not mandatory, but an option. It gives you more options if it’s available to them for any situations that arise that would be helpful to have it. I would venture to say for most visiting with parents, if the plebe or mid has town liberty, the whole point is to get off the Yard.

You will need a "pass" to enter most any military installation. The exceptions I am aware of are if you have Official Federal Government Law Enforcement Credentials, Military CAC Card, a Uniformed Services ID Card, a base decal, or are a contractor with pre-authorized pass. It is actually nice that USNA lets you pre-authorize visitors, most of the Academy's know parents and prospective students will be visiting at various times and they generally make accommodation for that, but sometimes it takes quite a while to get thru the checks and wait, it is kinda nice USNA lets you clear early get a pass then enter directly without having to wait in line.

The lines to get the DBIS card are long. We just dropped off our paperwork and picked up our card (much shorter wait and visit) when we came up for a game that Fall.
