Pay Forms and W-4

Is anyone else having trouble filling these out? Specifically the CG-2015, CG-2040s, and W-4 form. I can't even download the DD-2058 form, it just results in pdf file opening, claiming there's an error.

What kind of trouble are you having?

CG-2015 is where you will report information for the personal checking account that you want your pay delivered to. For all three CG-2040 forms, you just need to put your SSN/name at the top and then sign/date at the bottom...all other information has already been entered by Cadet Administration. These forms are authorizing them to withhold money from your pay and transfer it to the account listed on the form for the purpose identified.

I see the issue I had, for some reason, the info didn't previously provided info didnt print out. Thank you! Also, for the SSN, would I place that EMPLID box? I was massively confused from the example forms they gave us to work with
 
Just a tip on those pay forms....it may have changed in the past 3 years, but there was one form that said to include a copy of a voided check. Even though my daughter included a copy of a voided check with her forms, the swabs all needed the actual voided check when they arrived. We had to send her one in the mail. So have your swab take a physical voided check with them in addition to making the copy. Maybe they've changed the directions on that in the past couple years to avoid that problem.

That’s right! My son (‘25) asked us to mail him a voided check too!

That's correct. In the EMPLID box they pre-populated "SSN:" so put it there. Other than SSN, it's just name, signature and date. All the account numbers they need and everything else is prepopulated.

I agree it was very confusing. When I first saw these forms with names and account numbers, I thought they posted a form and failed to clear out some other persons info! lol

Has anyone figured out how to download the DD-2058 form? I tried on Mac, iPhone, and Chromebook but no luck. I also dowloaded a new PDF reader.

i was just going to ask the same thing. Do you all get a message suggesting to upgrade to latest adobe reader? I did that and still get the same message.

Its comforting to know I wasn't the only one with paperwork woes
 
All the forms downloaded fine when they were first posted. For fun, I just tried to download the DD-2058 form from the website and it's acting squirrely.
I've attached a copy of the blank form below for anyone who needs it. Hopefully it'll work this way.

Thank you for posting that form. That did the trick! I downloaded Acrobat Reader, but that didn't help. I also downloaded it to my computer. Maybe it's because I am using a Mac.

thank you! I had downloaded every version of adobe and still would not work. I think it might be a mac thing.

You can also clear your cache and try different browsers, including IE. Often govt systems are optimized for 1 browser more than others.

No, just complete yourself. It’s just name address, social, check box for single, sign and date.
Everyone who gets a job has to complete a W4. Like it talks about in the instructions, Unless you have multiple jobs and dependents (none of which can apply to you if you’re going to the academy), it’s just the basic info I mentioned.
