Permit To Report - Packet Question

Greetings friends:

We are in the process of overseeing our dd fill out her paper work in the permit to report packet.
Most of it is fairly strait forward.

But there are two questions that we have that I thought you all might be able to help us out with.

On the Midshipman Publicity Background Record card, there is a box that asks:

Type of appointment to USNA? By who? and District State?

What are the answers to these first two questions?
type of appointment? and by who?

Type of Appointment
They are looking for any one of;
"Congressional, Vice-Presidential, Navy & Marine Corps, ROTC, Child of Disabled Vet, Child of MOH recipient, Presidential"
(Where "Congressional" means either a Senator or Representative nominated you)

By Whom?

They are looking for your Nomination source: "John Q. Representative"

District State
They want to know the Congressional district number (in which you reside) and your state: "45 CA"
 
Speaking of the PTR package...some friendly advice. Make sure to make a copy of everything in that package and keep the package with you during I-Day. While your plebe will hand carry everything she needs as she walks through the Alumni Hall doors, I've read of more than one parent who received a call during the day for a copy of something that somehow got lost in the shuffle.

Good advice... while USNA is pretty good at the I-day process, it's tough to induct 1200 new Plebes in a day. On top of that , the stress meter is pegged for everyone -- USNA wants to do this just right, and Plebe is scared silly. Things will go wrong, and the the more prepared you are to address any situation, the less stress on everyone. However, you can't anticipate every contingency ...so don't sweat it. Two words will get you (both parent and plebe) through the day-- SEMPER GUMBY ! (Be flexible).
 
I don’t know if anyone else has had this problem, but my PTR did not include two forms (the parental driving and parking, and the alumni association forms), so I contacted my admissions counselor and he said that those are going to be mailed out separately, this month, and to not worry unless you don’t receive them before the end of May. Just some info in case anyone else has this problem..
