Permit to Report - patient registration form?

Hi all,

I’m a bit confused about one of the permit to report pages. For the “Patient Registration Form” (Medical Forms 2b), it is a one-page electronic pdf. However, this pdf says both “continued on back” and “FMP is explained on back.” Thoughts?

Just saw that the Patient Registration form now has two pages. We caught it right before the form was mailed.
