Personal Statement for DoDMERB

I need help with creating a personal statement for an Administrative AMI that is required before they can make a final determination for my physical examination. Is there a format I have to follow? Do I need medical records or am I telling them what happened from my point of view? And lastly, how do I go about submitting my personal statement to DoDMERB?
 
They should be specific in their request for what’s needed. Remedial codes, etc. Do you see any of that when you sign into DoDMERB? When DS was in remedial, it was listed there. He had to send everything to his assigned tech. If you click “contacts” or “Questions on the Process” on the left hand side, you should be able to find the email and phone numbers to your assigned tech. It goes by your last name. Other people on here are much more informed than I am, but this was just from my DS’s experience.
 
It says "R229.99 --- Does not require a physician's appointment". So does that mean I answer every question to the best of my ability? And if I am telling them, do I have to respond in a format similar to that of a business letter?
 
Did you get a letter attached? The “$$$” were specific in the letter correspondence. We had to send medical records associated with a surgery, no statement.
 
If they are asking specific questions. I would THINK answering each one to best of your ability would suffice and not have to be in a particular format, but I do not know. Sorry!
 
No particular format other than typical letter.
just say what you need to say. do not provide information not requested.
There is no telling what conclusions the reviewer will draw.
For example, "xyz happened but I was never officially diagnosed with abc".
That could trigger a remedial exam to determine if you abc, or something else.
DoDMERB is like court; LISTEN to the question, Answer the QUESTION.
If the statement is to say "yes I broke my thumb when I was 9 but have since played varsity basketball for 4 years....."
There is nothing to infer a deeper health issue.
