Police Record Check Form

My son and I went over this form which he will complete during spring break. Does he need to include where he is residing at college and all the cities we lived in since he was born or just from High School on? He's e mailing his admissions counselor but I am asking here.

We did local home PD and out of state High School town. I would assume that it would be required for local where his college is located.
