PPD Form?

In the PTR, There's a PPD (Mantoux Tuberculin Skin Test) form due according to the checklist. I couldn't find any form in the packet. Do I request such form from my doctor?

They just fill out the information from the TB Skin Test directly on the yellow form...I think it's about 1/3 way down on the form. The blanks to fill in are over on the right.

I just had them fill out the part on the yellow form. They didn't have another form.

PTR didn't come with Mantoux Tuberculin Skin Test form. DS had to get a PPD. I just attached the results form and Doc also annotated on the yellow form.
