PTR Immunization record and Physical?

I know the doctor needs to fill in and validate the immunization record that was received in the PTR envelope. However, the nurse seems to think she needs a physical. I have not seen any paperwork requiring an actual physical. Did I miss the paperwork for the physical or is it just the actual immunization records that are required to be filled in by the doctor? Her appointment is next week so I just wanted to get this clarified before she goes to the doctor.

Physical is not necessary. The medical staff just needs to administer a TB test and fill in the immunizations.
