Looks like noone has started a thread on this. . .

Does anyone have a link to the Patient Registration Form? You would think the instructions would have a link, but there is not, and I am not having luck finding it.

Go to the main permit to report screen and scroll down. Most of the links are available there. Including some updated notes in RED for issues with some of the links with specific browsers, etc.
 
What are u guys and ladies using for DoD ID number? Ssn ? Candidate number or blank for them to fill in?
 
I'd go with your SSN unless otherwise stated. Your DS/DD will get their CAC and DoD ID during Plebe Summer. Once given an ID, it is one of those numbers that is a good thing to memorize. You'll get asked for both your SSN and DoD ID many times. Also, putting your DoD ID into the Known Traveler Number box guarantees TSA pre-check when booking airline tickets.

Some paper does say candidate number other are not as direct. I think he said all needed sent asap he will ssn. The ones he will take he may leave blank for now
 
I know that the immunization record form must be completed by medical professional......does the DD Form 3111: Routine Immunization Screening Form also need to be completed by medical professional or just the individual and its brought with on I-day......

Plebe candidate fills that out and brings to I-Day
