Q to DQ with no change on PHQ

DD is a sponsored prep who was medically qualified last cycle for 4 academies and AROTC. She had to fill out a PHQ on which she indicated no changes occurred since being qualified. Back in September she received a remedial request for letter of explanation for mild sensitivity to heavily fragranced detergents (which was already addressed before being qualified first time around). She returned the letter and yesterday was shocked to get a DQ on her portal. We are absolutely floored since our understanding was unless there was a change indicated on PHQ the qualified date would simply be updated. Does anyone have experience with a situation like this? Did we misunderstand the process?
