Question about Dental Examination Form

My DS (2023 appointee) is filling out the Dental Form (Department of Defense Active Duty/Reserve/Guard/Civilian Forces Dental Examination). He wants to make sure to put accurate information in 3) Branch of Service, 4) Unit of Assignment, and 5) Unit Address. Is the following information correct: 3) Department of the Army, 4) USMA 5) BLDG.606, Thayer RD, West Point, NY10996

Any help would be appreciated!
 
That’s exactly what my son wrote in his and he now has a green check mark. Provide XRays requested and mail to them in the big white provided envelope usma will send you.
 
Does anyone have a green check next to (1) Immunization form and (2) Certificate of Authorization/Direct Deposit? If so, how long did it take to change? I mailed both two weeks ago. Haven't heard a peep (still red)

Immunization Form - e-mailed on 3/8, changed 3/18
Certificate of Authorization - mailed on 3/4, changed on 3/14
Actual Deposit - mailed on 3/11, still red

If the candidate is under 18 it will change to received for the immunization form but the circle will remain red until they are given the last vaccine on R Day. Confirmed that with cadet health services.
 
On the travel form, for Point of Contact is this the guardian/parent since it asks for the Guardian/Parent phone number below it and assuming so, is it also the guardian/parent email confirmation at the bottom of the form or the candidate email? This seems simple but we may be overthinking it here.
 
Need help finding someone to fill out my DS's Police Record form. We live in Maryland, and the county, state police will not fill it out. He doesnt have a school resource officer...not sure what to do next...

Also from Maryland. My kids go to private school, but the public school resource officer completed the form.
