Question about Eye Examination Form-Recommended Frame Size

My DS's optometrist at Costco said that she can't fill out the information for the Recommended Frame Size (#5). She said it should be done by the optical department. Can my son leave #5 blank or does he have to ask the optical department to fill that out? Since this form is signed by the Examiner, which is the optometrist, asking the optical department to do it doesn't look right. Has anyone had a similar experience?

I just looked at the numbers and filled it out on the form. You can bring it to the place that his glasses were obtained and verify quickly. They will wear the issued glasses during the beast. All the best.
