Questionnaire for NROTC issue

Hi, I received an NROTC scholarship, but I originally declined it because i wasn't going to a school with an NROTC unit, but I realized that was a mistake and tried to contact the NROTC office to change my decision, and I just checked this morning and they did change it, the only problem is, I have not mailed my questionnaire to the NROTC office, and I was supposed to submit it by May 8th, but I've been waiting on to see the status of my request to change my denial to acceptance since late April, plus I thought my mom mailed it when I received it in mid-April and I just found out this morning that she didn't, and in the scholarship information packet, it says the "nomination for the NROTC scholarship will be withdrawn." what exactly does this mean? What should I do? Thank you.
 
You should call Navy ROTC Command tomorrow morning (Monday, May 20th) first thing. They will provide all the information you need and answer all your questions. Please keep us posted on how you make out.

You are fine. If you sent in that request that means your declination was changed to accept. You do not need to send questionnaire back.
