Hi All,
I'm currently filling out my DoDMERB medical questionnaire and I'm just hoping to get clarification on question 135 and it's follow up.
The question is "Seen a psychiatrist, psychologist, social worker, counselor or other professional for any reason (inpatient or out-patient) including counseling or treatment for school, adjustment, family, marriage, divorce, depression, anxiety, or treatment of alcohol. Drug, or substance abuse (Applicant or recruiter will request sealed medical supporting documents from health care providers marked "CONFIDENTIAL: MEPS MEDICAL DEPARTMENT" and submit directly to MEPS medical personnel.)"
I saw a therapist for about a month, was never diagnosed with anything or received medication (I actually spoke with the therapist yesterday and they confirmed I had no diagnosis). Do I still need to request sealed medical documents like the question requires, and if so what would those documents end up being?
I already hit yes for the question and it locked me in, so if I unnecessarily hit yes I'm assuming I'd go into my appointment without e-signing the questionaaire and clarify to the doctor and they'd fix it then.
For the previous question which just asked about receiving counseling I selected yes, but for this question I'm just not sure how to proceed, or if it was such a small window of time with no diagnosis if it still falls under that term of "Professional Counseling".
Any help would be really appreciated! I've been stalking this form for a while now and there's always great people helping out, so I'm very thankful for that. I'm just not too sure how to proceed with this question.
