Questions about "Instructions to Appointees"

So far that is the only one I have noticed that prior service didn't have to fill out. You might be able to email the immunization clinic and see if they have access to those records or not. Not very hard to do though. I took DS, who's immunizations were up to date, in a about two weeks ago. He only needed the TB test and a polio as they were required to be within 6 months of entry into the Academy. The other paperwork was not very complicated, you may only have to update your SGLI or bring a current copy with you. Though you should check once again to make sure.

Immunizations are done prior to arrival?!?! Wow, cadets have an easy life. I was stuck multiple times during basic, and not always in the arm...

Not really though, I'm sure it's extremely challenging.
 
Well, being prior service myself, and on the enlisted side, I chose to make sure all of those shots were taken care of before my children got there. I am sure if you are missing anything, they do not hesitate to give you the shot right then and there. My children have/will carry a copy of their shot records through I-Day. Why should they be miserable and sore when it can be taken care of before hand?
Basic/BCT is challenging in different ways. A good percentage of the students getting off the bus for BCT will have left high school less than a month before and with the lack of communication will have lost their support network. They adapt quickly. Priors may not have as much struggle with BCT, but from what I have witnessed have a harder time during the regular year, and no not necessarily from the academics. They have to adjust to the idea that someone, maybe younger than them is giving them orders. They take a pay cut in most instances and loose a lot of freedoms to come and go as they please. So, IMHO all C4C's struggle to some extent, just in different ways. Good Luck:cool:
 
I've been working out and have started a diet plan to get in shape for BCT and my body shape is starting to change from when I received my uniform measurements. What should I do if in June I discover that some of my measurements have changed? :confused:

Don't worry too much USAFA will refit you on I day and give you new sizes.

I don't even know why they use that sheet, I think just to get an estimate of how many of each sizes to order. You get remeasured for everything, so don't worry too much about size changes. Some kids didn't even turn it in...
 
Question about USAFA form IMT O-143

My DS received BFE #2 in the mail today but there is some confusion about completing form USAFA IMT O-143, which is the record of Emergency Data for Cadet Personnel. There are two samples that accompany this form. The first sample shows the cadets name with upper case letters and lower case letters which showed, "Smith, John, Alexander"
My son completed his form using this format from this sample page.
However, on the second sample page, the first instruction says:
"Last name, first name, middle name....the name should be in all capitals..."
So, my son wrote his name using the first example with upper case as well as lower case letters. Then, the second sample sheet instructs cadets to use all capital letters. Now, we do not have an extra form and cannot figure out how to download a new form, and it is still unclear if using upper case and lower case letters on the cadet's name is right or wrong.
Did anyone else note the two different samples on the two separate pages?
Also, is this the kind of issue that will create problems for the cadets on "I-Day"?
Please let me know what you think?
 
batmom, we had the same issue last year. My son was so careful since the directions say no cross outs or white outs are allowed, and then to see 2 different sets of directions! From what my son could remember, it was turned in sometime that first day and no one really took the time to look at it much, other than to have him sign it, so the way your son did it should be fine. I'm sure there will be plenty of others that did the same thing. In the worst case scenario, a staff member might have your son rewrite it. My son didn't think this was anything the cadre (upperclassmen) ever saw, just a staff member who was processing this form.

You can fill out and print (or just print) the USAFA IMT O-143 here.
 
"Personal Debt" - Instructions to Appointees

My son has an appointment for the class of 2016 and has spent the last year at a private, four year college. He has a Federal Direct Unsubsidized loan in his name to pay for part of his tuition. Under the Instructions to Appointees, there is a statement that says, "Personal Debts: All personal debts must be settled before your arrival at the Academy." We are planning on clarifying this with admissions, but does anyone know if this refers to student loans? We were hoping to pay it off for him, but not all at once.

I'm curious about this as well. I just received the offer of appointment in the mail. One of the forms (USAFA FORM O-205) requires a signature from the official administering the oath on, I presume, I-Day? Do we take this form with us to inprocessing and sign it there? Or do we sign it now and send it back in?

That is a confusing form, my understanding is that it is not the form you sign at USAFA, it is simply a copy. Just bring it with you. You will sign another form with the oath on it when you arrive and inprocess on I-Day.

We had this exact situation 2 years ago. We simply stated in his paperwork that we were taking care of the debt. This worked fine and we did not have to pay it off all at once.

RE: Forms to complete to bring on I-Day?

Hi,
I am finding some of the requests regarding these forms unclear?
1)Immunization Record- it must be completed by the health professional (i.e. nurse or doctor) and faxed into the academy at 719-333-5448.
Then you must still bring a copy of this immunization record for I-Day.
2) You must have a copy of your eye glass prescription and must submit a copy to the USAFA at least 3 weeks before BCT. You CANNOT wear contact lenses at all during BCT.
3) You must complete Standard Form 86 questionnaire which is 127 pages long. This document must be signed in 4 places and brought to BCT for I-Day. I am not sure if they sign this document at I-Day or come with it already signed. Everyone also needs an original birth certificata, or passport etc.
4) Final school (transcript) grades must be sent to the USAFA as soon as school is done.
5) Form IMT-O-143 which is the Record of Emergency Data for Cadet Personnel must be completed and brought to I-Day. However, do NOT SIGN this form until I-Day when you and an officer will sign together.
6) SGLV 8286- which is the Serviceman's Group Life Insurance form which designates beneficiaries and the amount of life insurance you elect must be completed and sent in the the USAFA. However, I cannot tell if this form is signed first and sent or is it sent in without being signed. I have asked this question to the USAFA?
7) The cadet Sponsor program needs to be completed and signed ASAP.
8) The USAFA Assoc. of Graduates needs to be completed and signed ASAP.
9) The measurements for the uniforms need to be completed and sent in ASAP.
Anyone who can clarify what is signed in advance and what is not signed in advance- it would be very helpful.

batmom, I know we found many of the forms unclear last year--doesn't sound like they've made any improvements for this year! :shake:
1. Immunizations--its to your benefit to bring a copy just in case something you sent in gets lost--that way your cadet won't have to get unnecessary shots.
2. No contacts during BCT, but your cadet should bring them for when BCT is over.
3. Is this the security form? That has changed I guess, as the copy I have of my son's is only 18 pages long! He did sign it ahead of time.
4. Transcripts should be sent directly from school, but my son also took an official copy with seal etc. with him just in case.
5. Record of Emergency Data--sign at the Academy in front of a witness.
6. Insurance--sign at the Academy in front of a witness.
7, 8, 9 --Yes, go ahead and send in signed ASAP. Your son won't hear anything about a sponsor until after he's at the Academy.

Last year, for the things that my son didn't sign, he took a small brightly colored Post-It note and stuck it to the top of the form to remind him that it needed to be signed in case someone at the Academy forgot to check. He said it was a very hectic chaotic day (from an incoming cadet's perspective, maybe not from AFA staff!) so it was easy to forget all the things that needed to be signed, turned in etc. He kept all forms, copies, etc. in an expandable plastic envelope so it would be easy to find things that day.
