Remedial and private physician

I would have your private physician send the results directly to DoDMets via Fax, email, or whatever means is directed by the remedial letter. This will require you to sign an authorization of release for them to do so.
Additionally, I recommend obtaining a "hard-copy" for yourself in case you need to follow-up with DodMets and just for your records.

Where do I find the authorization of release form? Is this something the Dr has, or do I provide that document?
I know this is late but information may help someone currently needing it. Your doctor will have the release of information for you to sign basically authorizing him/her to release your information to DodMets.
 
I was looking at the Dodmets application and it said that if you use a different doctor, the results will be invalid. I would like to use a more local dr if possible. Did they recently change their policy regarding using your own doctor? Please advise.
You are given options: use DoDMERB recommended physician (preferred) or personal physician but you’ll incur the cost of the visit to your personal physician including bloodwork or any diagnostic tests
 
We did the "personal doc" thing with DS, DoDMERB accepted the results, but the SA did not like something about the exam, and asked DS to then go to Concorde's contract facility. Lost over two weeks of time. Apparently even though DS gave our physician the letter and printout from DoDMERB, something he did failed to satisfy the SA (have no idea if the exam failed to mention something, or part of the exam was not do how they wanted it) and they contacted DS and asked to have it done again. So not only did we have a delay in processing, but it cost us to have it done, and then we still had to travel to DoDmets facility anyway.

Won't happen all the time, but frustrating when it does. DS talked with DoDMERB and apparently if you do things on your own there CAN be problems, if Concorde does the remedial then apparently DoDMERB and the SA or ROTC program have no recourse. We don't understand it, but we can testify that it does happen. And DS can not find out why the initial remedial was not enough, it included everything they asked for.

And sometimes....you are not given an option. I read somewhere on one of these threads that some candidates are actually asked to personally visit the Waiver Authority for an in person exam. Check you remedials or waiver requests closely, you may not be able to use a private physician (I guess you can if you want, but it may not complete the process) unless the DoDMERB or WA says you can.
